Manhattan Life is somewhat unique in that it offers you self-enrollment tools for all of their products, which I have provided below. Self-enrollment can be helpful in cases where you would prefer to fill the application form yourself without giving too much information over the phone. As long as you enroll using the links below credit will still go to me, and i will still be your agent of record.

Affordable Choice

When I build out comprehensive Manhattan Life health plans, the Affordable Choice plan is the core INDEMNITY product that I start with.  It is an indemnity plan with fixed benefits, and FIRST DOLLAR coverage. What does FIRST DOLLAR coverage mean? It means there is no deductible, and there are no copays. FIRST DOLLAR means that the plan simply reimburses you for medical expenses.  For example, it could reimburse you $150 for a doctor's office visit (regardless of how much the doctor's office visit actually costs). You are not required to use in-network providers to get your reimbursement and file a claim. But to avoid submitting claims yourself, and to get network discounted rates on medical services, you'll want to be sure to use the First Health PPO nationwide network.  To provide more comprehensive medical coverage, the Affordable Choice plan is normally combined with other plans such as Accident, CHAS (Cancer, Heart Attack, Stroke), and other specified disease coverages.
Justin's Special Notes for Affordable Choice:
  • ✅ 12 month wait on pre-existing conditions
  • ✅ Hospital Observation pays 50% of Hospital Confinement (ask Justin for more details)
  • ✅ Only the hospital Accident benefit builds over time
  • ✅ Plan pays based off of the "Medicare Physicians Fee Schedule" which is state specific (1x, 2x, 2.5x, or 3x - depending on which Affordable Choice plan you choose.)
  • ✅ Doctors office visits are per calendar year, but up to 5 can roll over. Chiropractic is covered.
  • ✅ EKG / ECG falls under 'other radiology'
  • ✅ Plan does not pay for any injections, except for kids (vaccines and allergy only- $100 per year max)
  • ✅ Presription drug (Rx) benefit is per day not per prescription. Annual max for Rx is $750 per calendar year.
  • ✅ Note: Lab benefits are also per day, not per lab. This is one current downside to the plan.
  • ✅ Note: Ambulance only pays $150, so you might consider adding accident coverage to offset this low reimbursement rate!
  • ✅ Always make your effective date THE SAME as your billing date, otherwise you can be billed twice in the same month! They do not pro-rate.
  • ✅ Keep in mind that Manhattan Life products are MEANT to be BUNDLED!

CHAS - Cancer, Heart Attack, Stroke

Probably the second most important part of building a comprehensive medical plan with Manhattan life is the CHAS (Cancer, Heart Attack, and Stroke) coverage. I strongly recommend adding this coverage in case the worst should happen. CHAS pays a lump sum amount upon initial diagnosis with cancer, heart attack, and/or stroke. If the benefit amount paid exceeds your hospital bills then great news - you can keep the excess and put it in your pocket. This helps to defray the cost of taking time off from work, and keeps the bills paid while you recover.
Justin's Special Notes for CHAS:
  • ✅ One-time lump sum - pays out upon initial diagnosis of CHAS
  • ✅ Issue Ages 20–84, spousal discounts
  • ✅ Benefits can range from $5,000 to $75,000 (<$50k = easier underwriting)
  • ✅ Ideal to package with Affordable Care Act (ACA) and Medicare Advantage plans
  • ✅ There is a 30-day waiting period (following the effective date)
  • ✅ Optional riders include "Cancer Recurrence" (age 20–84) and "Heart and Stroke Restoration of Benefits" (age 20–54 only)
  • ✅ Note: Cancer is written separately from Heart Attack/Stroke. So if you've had cancer you can just purchase Heart Attack Stroke, and vice versa.
